Death of foster carer could not have been predicted or prevented

A significant case review published by Glasgow Child Protection committee relating to a boy called D who killed his foster carer, Dawn McKenzie, found some failings, however, concluded that her death could not have been either predicted or prevented.  The boy, who has been detained for seven years following the trial, had not shown any evidence of aggressive towards adults since he had been accommodated three years before though did have some known health issues.  The report did highlight indequate staffing and shortage of resources and the fact that staff did not have sufficient time to reflect on the complexity of the case within supervision.  The heavily redacted report is available to read on http://www.glasgowchildprotection.org.uk/
